Palliative Care
The plans are designed to work in tandem with curative treatments to help alleviate pain, nausea, insomnia and other uncomfortable symptoms of severe illnesses. The healthcare professionals help to integrate, administer and monitor any prescribed palliative care treatments.
Respite Care

Transitional Care
- Onsite access to transitional care coordination.
- Timely assessment of patient and family needs prior to discharge.
- Information and education for managing ongoing care, such as home care services, discharge plans, follow-up appointments and treatment.
- Support for hospital staff with home care and post-acute care referrals.
- Expedited referrals for all types of post-acute care including home healthcare, hospice care, palliative care, personal care and support, skilled nursing facilities and rehabilitation, specialized discharge needs (DME, Infusion, etc.)
- Improved transition to the home care team, from contacting physicians and coordinating prescription pick-ups to confirming start-of-care date and insurance authorizations.
- Insights on post-acute care trends and data based on readmissions by diagnosis.
Mental Health
